Sign Up Page
This is the sign up page for employees of companies that are enrolled in our Corporate Wellness Program. If your company has not yet enrolled please fill out the form at the bottom of the page.
There is no enrollment fee for this membership. Your monthly fee will be only $27. You will be paying for your first month's dues only. We will need to get your billing information when you come into the club for the first time.
